ABSTRACT

Background- The CNS space occupying lesions cause grave life-threatening outcomes irrespective of their nature as they grow in a confined space and are present close to vital structures Hence, it is of great importance to establish the accurate diagnosis for proper and timely neurosurgical intervention Tumours of central nervous system (CNS) are reported to be less than 2 % of all malignancies. In India, CNS tumours constitute about 1.9 % of all tumours. - AMethod retrospective study carried out in RIMS hospital for all the brain tissue specimen that have come for the histopathological examination, for a duration of one year (February2021 - January2022) among 78 cases of CNS lesions collected from archives of Department of Pathology. Astrocytoma was the most common entity followed by Meningioma whileResults- 2.56% cases were metastatic in this study sample. The present study helps to provide informationConclusion- regarding the disease burden in our area. This study attempts to categorise various CNS neoplasms as per recent WHO classification (2016) which has not only diagnostic implication but also has significant prognosis and predictive value

ABSTRACT

Nephrectomy, histopathological examination, neoplastic, renal cell carcinomaBackground: Nephrectomy either partial or radical has become a common procedure in surgical and urological practice. In this study pathological reports were interpreted for the following parameters: age, gender, pathological diagnosis. An indication of nephrectomy depends on type of lesion, extent of damage, general condition of patient and status of contralateral kidney. Method: The present ambiceptive data analysis has been carried out in Department of Pathology in tertiary care centre under Maharashtra University of Health Sciences by histopathological examination of nephrectomy specimens. A Results: total of 59 nephrectomy cases were included in the study. Non neoplastic and neoplastic cases were 44 (75%) and 15 (25%) respectively. Among non-neoplastic lesions, total of 32 cases (72%) were diagnosed as Chronic pyelonephritis with End Stage Renal Disease(ESRD) . Others Chronic Xanthogranulomatous Pyelonephritis with ESRD 9 cases (21%) and Chronic Glomerulonephritis with ESRD 3 cases (7%). Neoplastic lesions constitutes 15 cases with peak in 6th decade. Renal Cell Carcinoma(RCC) is the most common neoplastic lesion. The distribution among the neoplastic lesions include: Clear cell RCC (40%), Papillary RCC (13.34%), Mixed RCC (13.34%), Chromophobe RCC (13.34%), Multicystic nephroma (6.67%), Collecting duct carcinoma (6.67%) and Renal Medullary carcinoma (6.67%). Fuhrman's nuclear grade II is the most common among the RCC. Rare case such as Collecting Duct carcinoma, Renal Medullary carcinoma and Sarcomatoid RCC were diagnosed during the study period. On IHC, Collecting Duct carcinoma is positive for HMW CK, PAX8 and Renal medullary carcinoma is positive for Vimentin. The histopathological Conclusion: examination is essential to differentiate between benign and malignant neoplastic lesions and in grading of malignant lesions which help in treatment and further management of patients.

ABSTRACT

Background Current approach to detect the presence of gallbladder carcinoma involves a routine histopathological examination of all gallbladder specimens, regardless of the clinical characteristics of the patient or macroscopic aspect of the gallbladder. Available pathological reports were interpreted for the following parameters: age, gender, and pathological diagnosis. In addition, the demographic information and clinicopathologic characteristics of the patients with histopathologic findings were assessed in detail, and re-examination of the available specimens was done. Inpatient records of the Methods patients who had been operated for elective and emergency cholecystectomies in the hospital associated with Khaja Bandanawaz University – Faculty of medical Sciences; from January 2018 to November 2022, were retrospectively evaluated. Results A total of 661 gallbladder specimens submitted for histopathological examination during the study period were included in the study. The results of histopathological examination of these gallbladder specimens showed that chronic cholecystitis was found in 535 (81%), acute cholecystitis in 46 (6.99%), cholesterolosis in 44 (6.5%) patients, Dysplasia was found in 34 (5.14%) patients, and gallbladder carcinoma was detected in 2 (0.30%) patients. Conclusion A strategy of selective approach for histopathological examination of gallbladder specimens may be safe in areas with very low incidence of gallbladder carcinoma. Such selective strategy is more cost-effective, reduces the workload of pathologists, and does not appear to compromise patient outcome.

ABSTRACT

Background: Uterus is prone for various non-neoplastic and neoplastic lesions and also hysterectomy is the most common gynecological surgery indicated as a definitive treatment. Grossly unremarkable many specimens may reveal pathologies on histological examination. Hence, all hysterectomy specimens must undergo proper histopathological examination. In our study, hysterectomy specimens were studied and results compared with their clinical diagnosis. Aim and Objective: The objectives of the present study were to know the frequency and incidence of various lesions of the female genital tract and to compare findings of the present study with other studies. Materials and Methods: The study was a retrospective analysis of 250 hysterectomy specimens. All types of hysterectomies such as vaginal, abdominal, laparoscopic, and total abdominal hysterectomy without or with unilateral/bilateral salpingectomy/salpingo-oophorectomy were included in the study. Histopathological examination was done minimum by two faculty members. The results were noted and statistical analysis was done. Results: Total 250 hysterectomy specimens were studied. Vaginal hysterectomy was the most commonly performed type of hysterectomy which includes 187 cases (74.8%) out of 250 cases. Among the uterine pathologies, the most common lesion was leiomyoma (34.4%) followed by adenomyosis (26%). In our study during histopathological examination of cervix, the most frequent cervical lesion was found chronic cervicitis in 50.4% of cases. Conclusion: The present study highlights the wide spectrum of lesions of female genital tract and conveys that histopathology remains the primary modality for confirmation of diagnosis of lesions. We can conclude from the present study that many lesions had been diagnosed on microscopic examination though normal gross examination. Hysterectomy specimen should undergo proper histopathological examination, because it is the gold standard tool for final diagnosis.

ABSTRACT

Background: Appendix is a blind muscular tube derived from the midgut. During the embryogenic development, in the 6th week appendix and cecum appear as out pouching from midguts caudal limb. Acute appendicitis is one the most common diagnosis made whenever patient presents with right illac fossa pain and mostly encountered in young and middle-aged individuals. The diagnosis and management of acute appendicitis depend on clinical presentation, that is, clinical signs and symptoms. The most common presentations in these cases have abdominal pain along with fever, anorexia, nausea, and vomiting. Various Scoring systems are developed to diagnose acute appendicitis based mainly on the presenting signs and symptoms, but widely none of them are used as an uniform scoring system. Aims and Objectives: The purpose of this study was to assess effectiveness and comparison of modified Alvarado score and Tzanaki’s Score in the early diagnosis of acute appendicitis. Materials and Methods: A prospective observational study was done in department of General Surgery JLN Medical College Hospital, Ajmer which included 200 patients presenting with the signs and symptoms of acute appendicitis, clinically. During admission the patients were evaluated by Modified Alvarado score and Tzanaki’s Score and final surgical decision was taken by the treating surgeon. Finally, the scores were compared with the histopathological examination of the surgically operated specimen. Results: The sensitivity and specificity of Modified Alvarado Score was 84.26% and 72.7%, respectively, with a positive predictive value of 96.15% and negative predictive value of 36.3%. The sensitivity and specificity of Tzanaki’s score was 88.2% and 72.7%, respectively, with a positive predictive value of 96.31% and negative predictive value of 43.24%. The diagnostic accuracy of Alvarado score was 83% and that of Tzanaki’s score was 86.5%. Conclusions: Our study showed that Modified Alvarado scoring system is a simple tool with limited features for bedside diagnosis of acute appendicitis, but its effectiveness is not as good as that of Tzanaki’s scoring system.

ABSTRACT

Lipomas are the most common, subcutaneous, and slow-growing tumors composed of fat (adipose tissue). Out of many types of these benign tumors, ossifying lipoma (osteolipoma) is the rarest subtype. Here, randomly distributed trabeculae of lamellar bone are seen within mature adipose tissue. They are featured only as isolated case reports and small case series, thus the true incidence is not known. The most common site for osteolipoma is the head and neck region and they are rarely reported in the lower extremities. Herein, we discuss the case of a 19-year-old man who presented with a progressively enlarging painful mass in the left leg. Radiology was typical of osteoid osteoma. Excision was performed and histopathological examination confirmed the lesion as osteolipoma with no evidence of malignancy. No recurrence of the tumor was observed after 2 years of follow-up. Although osteolipoma is a rare entity, the differential diagnosis of the lesion can be kept in mind whenever we encounter ossification within the adipose tissue.

ABSTRACT

Introduction: Adnexal mass lesions are common among women which has a prevalence of 0.17%- 5.9% in asymptomatic women and 7.1%-12% in symptomatic women of all ages. Diagnosing the malignancy at early stage is difficult among adnexal masses. Early intervention and management can save many women in reproductive age group with good outcome. Aim: To find out the validity of Ultrasonography (USG) and Magnetic Resonance Imaging (MRI) in evaluation of uterine adnexal masses. Materials and methods: A Cross-sectional study of 45 subjects with suspected various adnexal masses over a period of one year from November 2019 to November 2020 in the Department of Radiodiagnosis and Obstetrics and Gynecology, were included. The data was entered in master chart then analysed using Statistical Package for the Social Sciences (SPSS) 19. Chi-square test has been used to compare the sensitivity of Tran abdominal Ultrasound (TAUS) and MRI scan with level of significance of <0.05. Predictive accuracy was measured using ROC curve. Results: The subject’s age ranged from 16 to 76 years with a mean age of 37.64. Ovary was the most common site of origin of adnexal masses. Most common adnexal masses on HPE were serious cystadenocarcinoma in malignant cases. USG showed an overall sensitivity of 58.3%, specificity of 100% PPV-100% and NPV-86.8% in comparison to the histopathological examination (HPE). Sensitivity, Specificity, PPV, NPV of MRI was 91.7%, 100%, 100% and 97.1%, respectivelyConclusion: USG is the initial choice of imaging modality for evaluation of adnexal mass lesions. But imaging with MRI has high accuracy in identifying the origin of a mass, characterising its tissue content, Vascularity, Septal Thicknessused in staging and preoperative plan. Sensitivity and diagnostic accuracy for MRI is higher than USG.

ABSTRACT

Acute cellular rejection (ACR) is a common complication after lung transplantation, which is mainly caused by the immune response of T lymphocytes recognizing the major histocompatibility complex on the cellular surface of grafts. It is currently considered as the main pattern of acute rejection. ACR is not only a direct cause of death of recipients, but also a high-risk factor for chronic rejection after lung transplantation. Nevertheless, it is a challenging task to deliver the diagnosis and treatment of ACR following lung transplantation. In this article, new progresses on the risk factors, pathogenesis, diagnosis and treatment of ACR in lung transplant recipients were summarized, aiming to improve the diagnostic and treatment efficiency of ACR and prolong the survival of recipients.

ABSTRACT

Background: The analysis of lymph node enlargement in the neck is not an easy task. These diseases which can be neoplastic also demands correct diagnosis for further management. The study intended to find out systematically the various pathological conditions presenting with enlarged lymph nodes in the neck, also the various modes of clinical presentation and behaviour of these conditions. Methods: The study population consisted of patients above 12 years presenting with cervical lymph node enlargement. The proforma which was drafted is used. Patient was examined systematically giving utmost importance to local examination. After making a clinical diagnosis, further relevant investigations were done to confirm the diagnosis. Treatment was instituted appropriately and followed up the patients. Results: Majority of the cases in this study had non-neoplastic causes for cervical lymphadenopathy in which tuberculosis is most common. Posterior triangle group of lymph nodes was most commonly affected in tuberculosis. Variable results were noted among the groups of lesions, with regard to local characteristics like number, laterality, mobility and involvement of other group of lymph nodes, etc. FNAC by virtue of it being inexpensive, quick in getting the results and easy to perform, is one of the important and essential diagnostic procedures. Conclusions: As cervical lymphadenopathy is an important disease, it always calls for meticulous attention, analysis and treatment. FNAC is found to be a frontline investigation of choice with biopsy and histopathological examination done for confirmation. Most of the non-neoplastic lesions are medically curable with limited role for surgery.

ABSTRACT

Breast lesions remain a major public health problem worldwide. Fine needle aspiration cytology (FNAC) has become one of the first-line investigations for the diagnosis of breast lumps. Although one of the major goals of FNAC is to differentiate benign from malignant lesions, in certain cases, this may not possible due to a lack of uniformity with regards to the reporting terminology used in breast cytology by pathologists worldwide, resulting in poor communication of results among health‑care providers. The present study aims to evaluate the role and diagnostic accuracy of FNAC in the evaluation of breast lesions using the National Cancer Institute (NCI) recommended terminology by correlating with histopathological examination (HPE) results.METHODSIn this retrospective study conducted over a period of two years, a total of 382 female patients with breast lesions underwent FNAC and was categorized according to the NCI guidelines. Of these, 156 cases had histopathological follow‑up and their FNAC diagnoses were compared. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) along with 95% confidence interval (95% CI) and accuracy of FNAC were calculated.RESULTSAmong the 156 cases, none were unsatisfactory (C1); 105 (67.1%) were benign (C2); 7 (4.4%) were atypical but probably benign (C3); 2 (1.1%) were suspicious favouring malignancy (C4); 43 (27.4%) were malignant (C5). Cyto-histopathological correlation was carried out. Of categories C2 and C3 (total of 112 cases), 109 were confirmed as benign (true negative) and the remaining 3 cases turned out to be malignant (false negative). Of categories C4 and C5 (total of 45 cases), all cases were confirmed as malignant (true positive) and none were benign (false positive). The present study showed a sensitivity, specificity, PPV, NPV and accuracy of 93.62% (95% CI, 82.46%–98.66%), 100% (95% CI, 96.67%–100%), 100% (95% CI, 92.5%–99.6%), 97.32% (95% CI, 92.4%–99.09%) and 98.08% respectively.CONCLUSIONSOur study concluded that FNAC is a rapid and effective method, and reporting of smears using NCI guidelines highly correlated with the histopathological diagnosis.

ABSTRACT

Bovine tuberculosis (BTB) is an infectious and zoonotic disease with socioeconomic importance, responsible for economic losses in livestock due to the decrease in animal productivity, mortality, bovine carcass condemnation and restriction on exportations. The objective of the present study was to evaluate the epidemiological aspects of this disease through a descriptive analysis and time series of cases of bovine tuberculosis diagnosed at the Laboratório Regional de Diagnóstico (LRD) of the Faculdade de Medicina Veterinária, Universidade Federal de Pelotas (UFPel) from 2000 to 2015. A survey was carried out in the LRD database of cases diagnosed as tuberculosis in cattle during this period. The incidence rate of BTB ([cases/population] x 100000) was calculated. Qualitative variables (sex, age, race, origin, type of material and presumptive diagnosis) were analyzed in a descriptive way, aiming to establish the profile of affected animals, characterize the municipalities in which the disease is originated and also fix the main referral organs for diagnosis. The time series analysis was performed using the Gretl statistical software 1.9.12 (GNU Regression, Econometric and Time-series Library). To verify the existence of the stationary distribution, the test of Dickey-Fuller was used, considering a value of p<0.05. During this period, 331 cases of BTB were diagnosed in the LRD. The present study identified that the predominant profile of the bovines affected by tuberculosis was female bovine aged more than two years old and crossbreed; the main organ sent for histopathological examination was lymph nodes. The microregion of Pelotas presented the highest incidence of the disease. There was also a tendency of a decrease in the incidence rate of this disease over the years, absence of seasonal influence in the occurrence of the disease and no cases of epidemics occurred from 2002 to 2015 in the area covered by the LRD-UFPel.(AU)

ABSTRACT

Idiopathic Granulomatous Mastitis (IGM) is an uncommon benign inflammatory disorder of breast. Peri-ductal mastitis is thought to be the initial inflicting factor for its development. IGM usually presents as painful swelling, abscess or sinus/fistula in breast. There are no specific findings on ultrasound or mammographic imaging, and moreover it looks BIRADS-IV on mammography. Histopathology provides the final diagnosis. Here authors report a case of bilateral IGM which was successfully treated with surgery.

ABSTRACT

@#Despite pulmonary cryptococcosis showing good response to antifungal treatment, making accurate diagnosis in a timely manner remains a challenge. Tissue for fungal culture is less sensitive, nonspecific, and time-consuming to allow for prompt diagnosis. We herein report a case of 31-year-old woman with no known medical illness but presented with history of cough and haemoptysis for two months. The chest X-ray and bronchoscopy revealed left lung mass. While tuberculosis showed negative result and no growth observed in the tissue culture, the histopathological examination (HPE) finding was suggestive of fungal infection. The formalin-fixed paraffin-embedded (FFPE) tissue was sent for molecular testing, which revealed Cryptococcus neoformans. This report emphasises on the advantages and limitations of polymerase chain reaction (PCR) as an alternative method to confirm the diagnosis in cases of culture-negative fungal infection.

ABSTRACT

Background: Adenomyosis and leiomyoma are the common causes of abnormal uterine bleeding (AUB). In this study it is aimed to evaluate the correlation of clinical and histopathological examination (HPE) of these entities leading to abnormal uterine bleeding. Methods: This retrospective study was carried out on hysterectomy specimens of subjects who presented themselves in the department of obstetrics and gynaecology of Sri Guru Ram Das Institute of medical sciences and research, Amritsar with chief complaints of AUB not responding to conservative treatment. Results: A total of 100 women with clinical diagnosis of AUB in which hysterectomies were performed, leiomyoma was found in 42% cases, adenomyosis in 22% cases. The most frequent combination of diagnosis was leiomyoma and adenomyosis i.e. 26%. In 9% cases chronic cervicitis and ovarian cyst were detected. In one case endometrial malignancy was found. Conclusions: Though adenomyosis and leiomyoma are clinically diagnosed along with other pathological conditions of the reproductive organs but their confirmation is still to be relied upon HPE; a most important investigation.

ABSTRACT

Background: Abnormal uterine bleeding is one of the most frequently encountered conditions in gynaecology practice and forms about 10% of all gynaecological admissions, the main concern in perimenopausal bleeding is that the bleeding could be the only external manifestation of many hidden serious pathologies of uterine-cavity. The objective of this study was to compare the results of transvaginal sonography (TVS) and drug and cosmetic (D and C) with histopathological examination (HPE) report of hysterectomy specimen in perimenopausal women with AUB.Methods: A prospective comparative study where 100 perimenopausal women with AUB were subjected to TVS then D and C and then the results were compared with histopathological report of the hysterectomized specimen. Results: With an endometrial thickness less than or equal to 15 mm the histopathology report is normal endometrium. When the endometrial thickness more than or equal to 15 mm the histopathology report is hyperplasia or carcinoma. Findings of TVS correlated well with histopathological report after hysterectomy. 14 cases of adenomyosis, 16 cases of myomatous polyp, and 6 cases of endometrial polyp missed by dilatation and curettage. Conclusions: TVS is a simple, non-invasive test to indirectly visualize the endometrial cavity and is useful as a first step diagnostic procedure in the evaluation of perimenopausal bleeding. Dilatation and curettage lags in detecting adenomyosis, endometrial and myomatous polyps, When TVS combined with dilatation and curettage, it can supplement the shortcomings of dilatation and curettage.

ABSTRACT

Background: Obstetric hysterectomy is done as a lifesaving procedure in very trying circumstances of life threatening severe hemorrhage. The study was undertaken with the aim to evaluate the relative frequency of hysterectomy done for obstetric indication among the hysterectomy specimens and also to assess the histopathological findings in the hysterectomy specimens. Methods: The study was conducted at a tertiary care center over a period of two and half years. Consecutive specimens of hysterectomy done for obstetrical indication were included. Gross and microscopic findings noted and data analysed. The study was approved by the Institute Ethical Committee. Results: Of the total hysterectomy specimens received obstetrical hysterectomy comprised only 1.3%(12/915) of all the hysterectomies. Patient’s age ranged from 20-36 years; mean 28.6 years. Parity ranged from 1 to 5; mean 2. More multiparous women 91.7%(11/12) had hysterectomies as compared to primiparous 8.3%(1/12) cases                         (p value <0.0001). All patients had single pregnancy. All (100%) patients underwent surgery through abdominal route with subtotal hysterectomy with preservation of the bilateral adnexae undertaken in most (11/12; 91.7% cases). About 5(41.6%) cases hysterectomies were performed after previous caesarean section and had abnormal placentation. Histopathological examination revealed adherent placenta in 33.4%(4/12), endometritis in 25%(3/12), rupture in 25%(3/12) and histologically unremarkable in 16.6%(2/12) cases. Conclusions: Obstetric hysterectomy is an emergency lifesaving procedure done in situations of uncontrolled post-partum hemorrhage. In recent years with more number of caesarean sections the incidence of abnormal placentation has drastically increased, thus making adherent placenta as the most common histopathological finding.

ABSTRACT

Respiratory pathologies are some of the common causes of deaths and it has been reported that in almost 1/3rd cases of sudden deaths the primary pathology may be attributable to pulmonary pathology. One of the important aspect of respiratory pathologies is that they can progress rapidly over a short period of time and in many cases these pathologies may prove to be fatal. In such cases gross and histopathological examination of lung tissue can give valuable insights into the cause of death. We conducted this prospective study to study the pathological findings in the lungs of such cases. Methods: The present study was conducted in the department of forensic medicine and toxicology of a tertiary care medical college situated in an urban area. The duration of study was 2 years. All cases of natural deaths brought for medico-legal autopsy were included in this study on the basis of a predefined inclusion and exclusion criteria. Virchow’s technique for dissection was followed in all the cases. The demographic details, gross and histopathological examination of lungs was done and findings were noted. Statistical analysis was done using SSPS 21.0 software. P value less than 0.05 was taken as statistically significant. Results: The study consisted of a total 54 patients out of which there were 34 males and 20 females with a M:F ratio of 1:0.58. The most common affected age group was found to be between 30-40 years (22.22%). The mean age of males and females were found to be comparable with no statistically significant difference (P>0.05).
